Death Investigation in Grant County

(Dry Ridge, Ky.) -- On June 25, 2013 at 1651 hours (4:51 p.m.) the Kentucky State Police responded to 617 North Main Street in Williamstown for a welfare check for a James M. Collinsworth (60). Mr. Collinsworth had contacted a family member advising he was having suicidal thoughts. Units arrived at the address at 1705 (5:05 p.m.) and made several attempts to make contact with anyone at the residence but were unable to. Units established at perimeter and again attempted to make contact with Mr. Collinsworth at 1737 (5:37 p.m.) at which time they heard a single gun shot from inside the residence. The units attempted several more times to make contact with anyone inside and were unable to. They then made entry into the residence and located Mr. Collinsworth with a self inflicted gun shot wound to the head. The Grant County Coroner pronounced Mr. Collinsworth deceased at the scene. The case remains under investigation by Trooper Kyle Reams. Trooper Reams was assisted by Trooper Jeremy Moore, Trooper Ryan Roessler, Trooper Gary Cooper, Sergeant Aaron Beighle, Lieutenant Tom Lilley, Members from the Williamstown Police Department and the Grant County Sheriffs Office.
