Injury Collision on US 41A in Hopkins County

Manitou, KY. (Hopkins County) - The Kentucky State Police investigated a single vehicle collision on US 41A, at the intersection of Wicks Well Road on Monday, October 6, 2014, at approximately 3:13 PM. Amanda Forsythe, 31, of Madisonville, KY was operating a 2000 Chevrolet Cavalier on US 41A at the intersection of Wicks Well Road. Dana Dacy, 50, of Dixon, KY was operating a 2006 Buick Rendezvous on US 41A northbound behind Amanda Forsythe. Ms. Forsythe stated that she was stopped in the northbound lane attempting to make a left turn into the Wicks Well Market. Ms. Dacy stated that she momentarily took her eyes off the road, causing the front bumper of her vehicle to make contact with the rear bumper of Amanda Forsythe's vehicle. The impact caused Ms. Forsythe's vehicle to exit the northbound shoulder of the roadway where it came to final rest in a ditch facing north. Ms. Dacy's vehicle came to final rest in the northbound lane of travel facing north. Kadenza Richey, 4, of Madisonville, KY and Destiny Mullins, 14, of Dawson Springs, KY were passengers in Amanda Forsythe's vehicle. Both passengers were transported by Med-Center Ambulance to Baptist Regional Medical Center for minor injuries. The collision is under investigation by Trooper Ben Sawyer. Trooper Sawyer was assisted at the scene by the Hopkins County Sheriff's Office, Manitou Fire Department, and Med-Center Ambulance.
