Two Vehicle Fatality Collision on US 41A

Dixon, KY (Webster County) - The Kentucky State Police is investigating a two-vehicle, fatality-collision that occurred on US 41A, approximately four miles north of Dixon, on Tuesday, October 27, at 1:54 p.m.

The preliminary investigation revealed that James L. Machen, 39, Providence, KY, was operating a 2009 Chevrolet Silverado northbound on US 41A. Rebbecca J. Slinker, 51, Clay, KY, was operating a 2006 Chevrolet Malibu southbound on US 41A.

As Mr. Machen was traveling northbound, his vehicle hydroplaned on the wet roadway and crossed into the southbound lane where it struck Ms. Slinker's vehicle at an angle. Both vehicles exited the roadway and struck an earthen embankment. Both vehicles came to rest off the roadway.

Mr. Machen was transported via private vehicle to Baptist Health Madisonville for treatment. Ms. Slinker and her passenger, Betty L. Cowan, 71, Clay, KY, were transported via ambulance to St. Mary's in Evansville where Ms. Cowan succumbed to her injuries. All occupants were wearing seatbelts.

Trooper First Class William Braden is investigating the collision. The Dixon Volunteer Fire Department, the Poole Volunteer Fire Department, Webster County EMS, and Providence EMS assisted at the scene.
