KSP Investigates Pike County Surface Mining Accident

(Phelps, KY) - The Kentucky State Police, Pikeville Post, is investigating a fatal accident that occurred at an Apex Mining site located in Phelps, KY. On May 28, 2015 Roy Mullins, 45 years old of Mouthcard, Kentucky, was attempting to attach a chain between a tractor trailer and grader when the accident occurred. Investigation at the scene determined that while working between the two pieces of machinery, the grader rolled back into the tractor, striking Mullins, who then became pinned between the heavy equipment. Mr. Mullins was pronounced deceased at the scene by Pike County Coroner Russell Roberts.

This incident remains under investigation by Trooper Jason McLellan. Trooper McLellan was assisted at the scene by State Mining inspector Gregory Goins, Phlelps Fire Department, and Transtar Ambulance Service.
