KSP Works a Fatality Collision in Lyon County

(Eddyville, KY) - The Kentucky State Police, Post 1 Mayfield, was notified by the Lyon County Sheriff's Office, of a two vehicle collision on March 23, 2016 at 2:09 P.M. on US62 near the Caldwell-Lyon County line.
Preliminary investigation shows that Michael J. Lefan, 49, of Eddyville, KY, was operating a 2001 Chevrolet pickup truck traveling eastbound on US62. Dennis W. Barlage, 61, of Effingham, IL was operating a 2016 Kenworth Semi Tractor Trailer traveling westbound on US62. For an unknown reason, Lefan crossed the centerline and traveled into the path of the oncoming Semi Tractor Trailer. Lefan was transported to Caldwell County Medical Center where he was pronounced dead by hospital doctors and the Caldwell County Coroner. Barlage was not injured in the collision.
Preliminary investigation shows that Lefan was not wearing his seat belt. Alcohol is not believed to be a factor in this collision.
Trooper Derek Scott, is continuing the death investigation. Trooper Jody Cash is conducting the collision reconstruction.
KSP was assisted by the Commercial Vehicle Enforcement division of the Kentucky State Police, Lyon County Sheriff's Office, Eddyville Police Department, Eddyville Fire Department, Kuttawa Fire Department, Lyon County EMS, Lyon County Rescue, and the Caldwell County Coroner's Office.
