KSP Works Fatal Collision in Lyon County

Kuttawa, Ky. (September 3, 2016) On September 3, 2016, at 1:56 p.m., Kentucky State Police post 1 Mayfield received a 911 call regarding a two vehicle collision at the 37 mile marker near Kuttawa, KY.

Preliminary investigation shows that Leslie D. Cunningham, 71, of Evansville, IN was operating a gray 2015 Subaru WRX traveling westbound in the right hand lane of Interstate 24. Joseph M. Mouser, 73, of Rineyville, KY was operating a gray 2007 Ford F150 traveling west bound in the left hand lane just beside Cunningham's Subaru. Cunningham turned left into the path of Mouser's F150 pickup in what appears to be an attempt to make a U-turn across Interstate 24. Mouser could not avoid striking Cunningham's vehicle in the driver side.

Mary D. Fugate, 72, of Louisville, KY was a passenger in Cunningham's vehicle. She sustained multiple blunt force injuries and was pronounced dead at the scene by the Lyon County Coroner.

Cunningham was air lifted from the scene to Deaconess Hospital in Evansville, IN with non-life threatening injuries.

Mouser sustained some minor abrasions on his arm but refused medical treatment.

The investigation and collision reconstruction is ongoing by Detective Eric Fields.

KSP was assisted on scene by the Lyon County Sheriff's Office, Kuttawa Fire Department, Lyon County Rescue, Lyon County EMS, Lyon County Coroner, Air Evac and the Kentucky Transportation Department.
