Kentucky State Reformatory Inmate Dies in Hostage Situation

LaGrange Ky. (December 28, 2016) – (Update on death of inmate at Kentucky State Reformatory)
The Kentucky State Police at Post 5 Campbellsburg is investigating the death of an inmate in reference to an officer involved shooting at the Kentucky State Reformatory in LaGrange KY.
At approximately 6:48 p.m. on December 27, 2016, Kentucky State Police in Campbellsburg received a call from the Kentucky State Reformatory about a hostage situation. An inmate, David Carver, allegedly took a nurse employed by Correct Care Solutions hostage with an improvised edged weapon. Troopers trained in hostage negotiations responded to the scene. Troopers attempted to negotiate Carver’s surrender for approximately 2 hours.
As negotiations began to deteriorate, Kentucky State Reformatory’s Correction Emergency Response (CERT) Team was activated and assumed control of the situation. Troopers then cleared the scene and exited the prison. The CERT team continued negotiations until the inmate made an aggressive move towards a member of the CERT team with the edged weapon. The CERT team member then discharged his issued firearm, striking Carver. Carver was transported to Baptist Health in LaGrange by ambulance where he was pronounced deceased by the Oldham County Coroner’s office at approximately 7:25 p.m. Next of kin was notified by the Department of Corrections with the assistance of Troopers from Post 2 Madisonville.
