Greenville, Ky. (Muhlenberg County) – On April 22, 2022, Trooper Jacob Stephens presented his investigation to the Muhlenberg County Grand Jury. The grand jury returned indictment warrants and Kaitlyn Piper was subsequently arrested and charged with Murder, Assault 1st degree, DUI 1st offense (aggravating circumstances), and persistent felony offender 2nd degree. She was lodged in the Muhlenberg County Detention Center.

Greenville, Ky. (Muhlenberg County) – On Friday, April 30, 2021, around 10:58 AM, the Kentucky State Police Post 2, Madisonville, was requested by the Muhlenberg County Sheriff’s Department to investigate a fatal collision on KY 189.
Preliminary investigation revealed 25-year-old Kaitlyn Piper, of Beech Creek, KY, was operating a 2004 Mercury Grand Marquis Northbound on KY 189. For an unknown reason, near Greens Chapel Road the vehicle drifted across the Southbound lane and continued traveling off the Southbound shoulder of the roadway before striking a tree. None of the occupants were properly restrained. Kaitlyn Piper and a juvenile passenger were transported by helicopter to Vanderbilt Health in Nashville, TN for their injuries.
Another juvenile passenger, Serenity O’Bannon, age 1, of Beech Creek, KY, was transported by ambulance to the Muhlenberg Community Hospital where she succumbed to her injuries and was pronounced deceased by the Muhlenberg County Coroner. The investigation is ongoing by Trooper Jacob Stephens.
