Madisonville, KY (Hopkins County) – At approximately 23:44 hours on 09/05/2021, Kentucky State Police Post 2 was requested by the Hopkins County Sheriff’s Office to investigate a two-vehicle collision that caused multiple injuries and one fatality. Troopers responded to approximately the 111 mile-marker of Interstate 69 northbound to begin the investigation.
Preliminary investigation revealed a 2013 Chevrolet Malibu, being operated by Mr. Horazeon Boyd 21, of Paducah, Kentucky was traveling north in the right lane when for an unknown reason, his vehicle slowed in the travel portion of the roadway. Upon slowing in the roadway, Mr. Boyd’s vehicle was struck in the rear by a 2018 Dodge Caravan, being operated by a Ms. Whitney Gibson, 24, of Madisonville, Kentucky. After colliding, both vehicles left the east side of the roadway and came to final rest in a field.
Mr. Boyd, his front-seat passenger, Brianna Teague, and right-rear passenger, Briantavion Haynes were transported from the scene by ambulance; Mr. Boyd and Mr. Haynes were later transported via helicopter to Saint Vincent Hospital in Indiana. Hopkins County Coroner, Dennis Mayfield, pronounced the left-rear passenger, Destinee Scott, 18, of Paducah, Kentucky deceased on scene.
Whitney Gibson, her front-seat passenger, William Nash, and an infant passenger were transported from the scene by ambulance to Baptist Health – Madisonville.
Kentucky State Police Post 2 was assisted on scene by the Hopkins County Sheriff’s Office, Madisonville Fire Department, Grapevine Volunteer Fire Department, PHI Air Medical, Medical Center Ambulance Service, and the Kentucky Transportation Cabinet.
Interstate 69 was closed for approximately four hours as Kentucky State Police investigated the collision.
